Parkway Kids Camp - Week #2

Camp mornings are filled with songs, Bible stories, games, crafts, silliness, friendships and more! Camp runs from 8:45am-12:30pm for children who have completed JK - grade 5. Campers need to bring a labelled water bottle and sunscreen; snack will be provided.

IMPORTANT:

**If registering MULTIPLE CHILDREN for a week of camp, it is imperative that you select the # of tickets (= to # of children) you will need for your family.

***PLEASE NOTE* that the programming is the same for both weeks of camp. Each camper can only be signed up for one week of camp to allow others an opportunity to attend. We can't wait to have a fun and sun-filled summer with you all!

I, as the camper's guardian, hereby authorize Parkway Church to photograph myself, and my family (including minors), and any work or actions performed by the said individuals during and for church events.
I understand the photos may be used for social media platforms, promotional material, and instructional purposes only and will not be used to generate a profit or any other commercial purposes. I understand that I will not be compensated nor will I seek compensation for the use of said photos. I release Parkway Pentecostal Church from responsibility should a third party violate the terms of this release.

Liability Waiver

While every precaution is taken for the safety and good health, some sports and activities carry with them the inherent risk of personal injury beyond the risks associated with many of the recreational activities at Parkway Pentecostal Church. I/we understand and accept these risks and agree that by allowing my child to participate in those activities, he/she may be taking part in a recreational activity that presents the potential for personal injury.

I/we the parents or guardians named below, authorize the Pastor or one of Parkway Pentecostal Church ministry personnel to sign a consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.

I/We, named below, undertake and agree to indemnify and hold blameless Parkway Pentecostal Church and its personnel, it's Pastors and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Parkway Pentecostal Church and as well as of any medical treatment authorized by the supervising individuals representing Parkway Pentecostal Church. This consent and authorization is effective only when participating in or traveling to events of Parkway Pentecostal Church.

In the case of possession of dangerous items or non-compliant behavior, at the discretion of the Youth Pastor, parents/guardians will be contacted and required to pick up their child immediately.
